About post-hospital recovery care
Recovery doesn’t end when you leave the hospital.
Leaving the hospital is an important step, but it doesn’t always mean someone is ready to manage everyday life alone. Many people need temporary help with daily routines, mobility, meals, and household tasks while they continue healing at home. Family members may suddenly be balancing caregiving with work, children, and other responsibilities.
At PrimeCare In-Home Support, our post-hospital recovery care bridges the gap between hospital and home. Our caregivers offer personalized, non-medical support that promotes independence and gives families peace of mind, while your loved one follows their healthcare provider’s recovery plan. It is part of our specialized care services.
Whether the recovery follows surgery, an illness, an injury, or an extended hospital stay, our team is here to provide dependable care when it matters most.

How we support post-hospital recovery care
Comfort, routine, and confidence at home.
Recovering in a familiar place can bring comfort, reduce stress, and let your loved one focus on rebuilding strength close to family.
Consistent daily routines
Our caregivers help set up routines that encourage safety, comfort, and confidence from the first day home.
Alongside your care team
Our support complements the guidance of physicians and rehabilitation professionals. It never replaces it.
Non-medical by design
Caregivers assist with daily living, safety, and comfort. They do not provide clinical or medical treatment.
Planning the transition
Ask the right questions before discharge.
A little preparation makes the first weeks at home easier. AHRQ’s Taking Care of Myself guide and Medicare’s discharge planning checklist help patients and families prepare to leave the hospital.
- Know your follow-up appointments and medications
- Ask what help your loved one will need at home
- Arrange meals, transportation, and a safe walking path
- Line up extra support before the first night home
Family Caregiver Alliance also offers a hospital discharge planning guide for families. When you’re ready, we’ll help you put the home care side in place.

Track appointments, medications, and rehabilitation visits in one place. Our caregivers can provide transportation to follow-up visits.

What is post-hospital recovery care?
Post-hospital recovery care is temporary, non-medical help at home after a hospital stay. Caregivers assist with bathing and dressing, mobility, meals, medication reminders, housekeeping, errands, and companionship while your loved one follows their healthcare provider’s recovery plan.
Is post-hospital recovery care medical care?
No. PrimeCare provides non-medical care. Our caregivers support daily living, safety, and comfort, but they do not provide clinical or medical treatment and do not replace guidance from physicians or rehabilitation professionals.
Who can benefit from post-hospital home care?
It may help someone returning home after surgery, a stroke or other neurological event, a joint replacement, an illness requiring hospitalization, an injury, a rehabilitation stay, a cardiac procedure, a respiratory condition, or an extended hospital admission.
How soon can care start after discharge?
Call us as early as you can, even before discharge, so we can plan the transition. We will schedule your free in-home assessment and start care as soon as a matched caregiver is available.
Can you help with follow-up appointments and rehabilitation visits?
Yes. Caregivers can provide transportation to follow-up appointments and rehabilitation. Learn more about our transportation service.
How long will my loved one need care?
Every recovery is different. Some families need a few weeks of support and others need longer. We adjust the care plan as independence returns, so you only receive the help you need.
Can post-hospital recovery care include overnight or 24-hour support?
Yes. Some people need extra help during the first nights home. Ask us about overnight care or 24-hour care, and we’ll recommend what fits at your free assessment.

Cost depends on the schedule and level of care. PrimeCare offers flexible scheduling with a 4-hour minimum shift. See our billing page or contact our team for a personalized quote.
