About specialized care
Every condition changes what a good day looks like.
Living with dementia, Parkinson’s, or recovery after a stroke or hospital stay brings needs that general help doesn’t always meet. The right support depends on the condition, the stage, and the person’s own routines and preferences.
At PrimeCare In-Home Support, our specialized care is non-medical and built around the individual, not the diagnosis. It works alongside your loved one’s doctors and nurses, helping them stay safe, comfortable, and independent at home across Alameda, Contra Costa, San Mateo, San Francisco, and Santa Clara counties.

What is specialized home care?
Specialized home care is non-medical support tailored to a specific condition or situation, such as dementia, Alzheimer’s, Parkinson’s, stroke recovery, post-hospital recovery, or palliative support. The care plan reflects the condition, its stage, and the person’s routines and preferences.
Is specialized care medical care?
No. PrimeCare provides non-medical care. Our caregivers help with daily living, safety, comfort, and companionship, and they work alongside your loved one’s doctors, nurses, and therapists rather than replacing them.
What is the difference between dementia care and Alzheimer’s care?
Dementia is the broad term for a decline in memory and thinking, and Alzheimer’s disease is the most common cause. Both call for patient, routine-based support, and we tailor the plan to the person’s diagnosis, stage, and needs.
Can we combine services, such as dementia care with overnight care?
Yes. Many families combine services, for example dementia care with overnight care or respite care. During the free assessment we build one plan and schedule around your loved one’s needs.
Who provides specialized care at PrimeCare?
Every caregiver is a Registered Home Care Aide with the CDSS and completes fingerprinting and a full criminal background check before working with clients.
How many hours of care can we schedule?
PrimeCare offers flexible scheduling with a 4-hour minimum shift, from a few shifts a week up to 24-hour care. Contact our team to build a schedule around your family’s needs.
